PLEASE FOLLOW INSTRUCTIONS CAREFULLY:
Hello, Harriet Tubman Charter School Parents! Complete your child’s enrollment NOW to confirm your child’s slot! **Avoid being waitlisted!
Please do the following ASAP:
1. GATHER the following documents:
- Birth Certificate
- GA Immunization Certificate (Form 3231)
- Vision/Hearing/Dental Nurtition Certificate (Form 3300)
- Government Issued Photo ID of Parent/Guardian (DL/State ID)
- Special Education IEP/Gifted/Speech/Title 1/ESOL/ELL Program Copies (not required)
- Standardized Test Scores (not required)
- Child's Social Security Card
- Proof of Guardianship
- Previous Report Card (not required)
- Proof of Residency Documents (Recent utility bill, lease, property tax receipt, voter registration card insurance card -- Please provide 2)
- Student Media Release Form
2. UPLOAD by taking a clear picture of your documents with your phone OR scan docs to a computer to upload.
3. SIGN the enrollment form and upload ALL required documents.
4. CONFIRMATION of completed enrollment will be emailed within 48-72 hours.
**You will have 5 days to complete enrollment to secure your child’s position in our lottery.
