Post-Lottery Enrollment Procedures for Fall 2021

Hello, Harriet Tubman School of Science & Tech Parents! 

**Please note that if you would like a paper application, you may stop by the school’s main office Monday-Friday, between the hours of 2:15 p.m. and 4:15 pm. To contact our office, please call (678) 705-2307.**


1. GATHER the following documents:

  • Birth Certificate

  • GA Immunization Certificate (Form 3231)

  • Vision/Hearing/Dental Nutrition Certificate (Form 3300)

  • Government Issued Photo ID of Parent/Guardian (DL/State ID)

  • Special Education IEP/Gifted/Speech/Title 1/ESOL/ELL Program Copies (not required)

  • Standardized Test Scores (not required)

  • Child's Social Security Card

  • Proof of Guardianship

  • Previous Report Card (not required)

  • Proof of Residency Documents (Recent utility bill, lease, property tax receipt, voter registration card insurance card -- Please provide 2); If living with someone in the district, but not on lease, please provide a notarized letter signed by both parties

  • Student Media Release Form


2. UPLOAD by taking a clear picture of your documents with your phone OR scan docs to a computer to upload.


3. SIGN the enrollment acceptance form and upload ALL required documents.


4. CONFIRMATION of completed enrollment will be emailed within 48-72 hours.


**You will have 10 days to complete enrollment to secure your child’s position in our lottery. 


**Technology Tip: You can download the ‘Genius Scan’ app to your phone for FREE & use it as a scanner.

**Students will be asked for a social security number at the time of enrollment. The lack of or objection to providing a social security number will not prevent the student from being enrolled.  Students should not be denied enrollment due to lack of a social security number. Parents and legal guardian may sign the following form to exempt them from providing a social security number: Social Security Number Waiver Form
